When you arrive for your first visit, please be prepared to complete all insurance and health information forms that will allow us to begin your dental treatment. We will also request you fill out several forms that will help us get acquainted with your previous dental history and concerns.

Your initial exam will last approximately one hour. We provide an evaluation that will outline your existing dental problems, proposed treatment as well as oral hygiene maintenance instructions to help you care for your teeth and gums. You will be given a treatment plan, including fees for any proposed treatment. You can be assured that we will never “perform” treatment until you have been” informed” of the fees as well as the procedures.
